Growing together.\n\nAs a Registered Nurse Clinical Care Coordinator, you will play a vital role in our Integrated Care Model by assessing members' needs, advocating for optimal care, and ensuring they have access to the services required.\n\nThis is primarily a field-based position where you will spend at least 50% of your time visiting members in their homes or long-term care facilities. Flexibility, adaptability, and patience are key qualities needed for this role.\n\nYour Primary Responsibilities will include : \n
- Engaging with members both in-person and via phone to conduct comprehensive assessments that cover medical, behavioral, functional, cultural, and socioeconomic needs.\n
- Developing and implementing individualized care strategies aimed at providing the appropriate level of care.\n
- Identifying and facilitating referrals to social service programs to meet financial, psychosocial, and community support needs.\n
- Managing care plans continuously and acting as a single point of contact throughout the care process.\n
- Collaborating with internal teams, healthcare providers, and community resources to ensure effective care implementation.\n

- Current and unrestricted independent licensure as a Registered Nurse.\n
- Two or more years of clinical experience.\n
- Intermediate proficiency with MS Office tools, including Word, Excel, and Outlook.\n
- Dedicated workspace at home to ensure privacy for Protected Health Information (PHI).\n
- Ability to travel within assigned areas, visiting members in various settings like their homes and community centers.\n
- Reliable transportation and a valid US driver's license.\n\nPreferred Qualifications : \n
- Bachelor's or Master's Degree in Nursing.\n
- Certified Care Manager (CCM) credentials.\n
- Community case management experience, particularly with individuals having complex needs.\n
- Experience in Managed Care settings.\n
- Team-based care experience.\n
- Proficiency in using Electronic Medical Records or other electronic systems.\n
- Bilingual abilities, especially in Spanish.\n\nPhysical Requirements : \n
- Ability to transition between office and field locations throughout the day.\n
- Capability to navigate different terrains and visit various healthcare providers and members.\n
- Capacity to transport equipment such as laptops for field visits.\n
