About The Position
Title : Licensed Independent Clinical Social Worker
Location : Bellevue, WA
Duration : 13 Weeks
Shift : Night – 5x8-Hour (20 : 30 – 07 : 00)
Job Responsibilities :
- Participate in daily interdisciplinary rounds and contribute to care planning.
- Maintain current knowledge of patients’ psychosocial status and discharge plans.
- Act as liaison between patients, families / support systems, and outpatient providers.
- Review mental health histories and identify potential risks or chart flags such as mental health directives, firearm access concerns, and family / provider contact requests.
- Conduct psychosocial assessments within 72 hours of admission and identify ongoing care needs.
- Coordinate discharge plans ensuring CMS-compliant follow-up appointments and adequate medication supply.
- Facilitate referrals to outpatient and community-based programs, ensuring all releases of information (ROIs) are properly documented.
- Independently complete mandated reporting to APS or CPS when required.
- Assist with discharge logistics, including transportation coordination and community resources.
- Communicate effectively with interdisciplinary teams and document all updates using approved tools and templates.
Required Qualifications :
Master’s Degree in Social Work (MSW) from an accredited institution.Licensed or license-eligible in Washington State (LICSW / LSWAIC preferred).Minimum 2 years of experience in clinical social work.Knowledge of trauma-informed care, community resources, and safety planning.Familiarity with CMS documentation and regulatory standards.Excellent communication, organization, and crisis management skills.Ability to collaborate effectively in a fast-paced, interdisciplinary environment.EPIC charting experience required.10907817EXPPLAT
