Key Responsibilities
Manage and coordinate patient care in both in-person and telephonic settings
Conduct assessments and develop care plans for patients
Collaborate with healthcare teams to ensure quality care delivery
Required Qualifications
Active and unrestricted LPN license in Ohio or a compact state
3-5 years of experience in case management, home health, or waiver services
Behavioral health experience is preferred
Proficiency in MS Office Suite (TEAMS, Word, Excel, Outlook)
Strong computer skills for tasks such as attaching files and sharing screens
