Behavioral Health-Patient Navigator

Location-Pahoa

Pay Rate-$24.15-$38.65 (Compensation will be determined based on qualifications and experience)

About the Role :

The Behavioral Health Patient Navigator assesses patient needs, creates service plans, connects them to community resources, and collaborates with HICHC departments and partners to address social determinants of health.

Minimum Qualifications :

• Required -Bachelor’s degree in relevant human services field plus at least one-year experience required; OR any equivalent combination of experience, training and / or education.
• Preferred- Staff are expected to be Tobacco Treatment Specialist (TTS) eligible (meaning a non-nicotine user for at least 6 months) or hold TTS certification (
• Preferred -Previous experience working in a team environment, clinical experience working with diverse clinical populations and knowledge of the Hawaiʻi Island community
• Preferred- Eligible for future licensure in Social Work, Counseling, or other related field
