Oregon Licensed Care Manager (RN)

A company is looking for a Care Manager (RN) specializing in Pregnancy & Postpartum.

Key Responsibilities

Develop and facilitate personalized care plans for members, focusing on physical needs and social determinants of health

Coordinate care between members, families, and providers to ensure access to appropriate services

Monitor member progress and adjust care plans as necessary, providing education and resource support

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in care management

Registered Nurse (RN) license in the state of Oregon

Preferred experience in OB / Labor & Delivery or pregnancy and postpartum programs
