Working Title : Self Determination Case Manager

A Motor Vehicle Records screening will be conducted prior to employment.

This position is designed as Hybrid (In office and Remote).

This position requires a criminal background check. Therefore, you may be required to provide information about your criminal history in order to be considered for this position.

Qualifications

Education and Experience : Graduation from an accredited college or university with a bachelor's degree in special education, social work, rehabilitation counseling, nursing, occupational therapy, physical therapy, recreational therapy, vocational rehabilitation, sociology, psychology or other behavioral science or human services field and experience equivalent to one year of intellectual or developmental disabilities program or staff work.

Substitution of Experience for Education : Qualifying developmental disabilities, social services, nursing, therapy, behavioral, or mental health work may substitute for the required education on a year-for-year basis to a maximum of four years.

Substitution of Education for Experience : Graduate coursework in special education, social work, rehabilitation counseling, nursing, occupational therapy, physical therapy, recreational therapy, vocational rehabilitation, sociology, psychology or other behavioral science or human services field may be substituted for the required experience on a year-for-year basis to a maximum of one year.

Necessary Special Qualifications : Some positions require that applicant be a qualified intellectual and developmental disabilities professional under ICF / IDD Federal regulations.

Overview

Under general supervision, this classification is responsible for professional intellectual and developmental disabilities program coordination of average difficulty; and performs related work as required. An employee in this class is responsible for individual support plan development, service planning and coordination, and assessment of ongoing programmatic and financial eligibility for a moderate-sized group of service recipients.

Responsibilities

• Uses relevant resources to make determinations in order to comply with current guidelines. Reviews and follows federal, state, and departmental regulations to assure compliance.
• Obtains information to determine eligibility of services and to plan services for individuals. Makes recommendations and decisions on level of need based on individual's circumstances and schedules face-to-face visits in the home and in the community.
• Mentors new employees to help them understand departmental processes.
• Estimates the projected annual budget for services to comply with federal regulations. Compiles and audits relevant information from all sources to develop individual support plans, monthly reviews, and to ensure implementation of individual support plans.
• Makes referrals to outside organizations to ensure that the individual's needs are met. Creates, updates, and maintains current individual support plans, documentation of contacts, and monthly review notes to assure service delivery and compliance with current federal, state, and departmental policy.
• Facilitates discussion of alternatives during conflicts and / or disagreements between members of planning teams. Identifies individual-specific changes to plan services that meet the needs of the individual through face-to-face visits, phone calls, reviewing medical information, and talking to direct care staff and family members.
• Provides accurate, concise information so that the work team can evaluate and coordinate services. Utilizes person-centered planning tools to develop individual support plans for individuals.
• Develops and maintains constructive and cooperative working relationships with others by incorporating ideas, providing feedback and follow-up, seeking input, and finding solutions to problems. Provides written materials to and explains them as needed to individuals, families, conservators, advocates, and other state and community agencies so that everyone has a common understanding of services and supports.
