PRN Hospice RN– Dexter

What You’ll Do

• Provide high-quality nursing care in patient homes
• Complete assessments and create personalized care plans
• Monitor and respond to changes in patient conditions
• Partner with physicians and the interdisciplinary team to coordinate care
• Educate patients and families on comfort care, medications, and disease progression
• Support families with compassion through the end-of-life journey
• Ensure compliance with physician orders and care standards
