Responsibilities
Assures that all consumers receive clinically sound triage / referral and ongoing consumer services for behavioral health needs.Collects information necessary to develop an accurate assessment of each consumerParticipates in the development of the consumer treatment plan.Provides mentoring, individual and group support, and teaches self-advocacy and motivation through feedback.Provides community outreach and sustained efforts to engage individual clients.Provides linkage to mental health services and programs including but not limited to psychiatric services, medication management, and representative payee.Evaluate the consumer's treatment plan through follow-up assessment and recommends adjustments based on clinical findings.Qualifications
Education :
Requires GED or High School DiplomaCompletion of, or willingness to complete within 6 months (after date of hire) approved Peer Support Training ModelMust have lived experience with a mental health, or behavioral health condition and in recoveryBe willing to apporiately share elements of their experience with others in a professional and supportive mannerLicense(s) / Certification(s) :
Crisis Prevention Institute (CPI) training within 6 months of hire.Valid driver's license when driving any vehicle for work-related reasons.Experience :
1 year of experience in providing mental health or human servicesSpecific offers are determined by various factors , such as experience, skills, internal equity, and other business needs.
