EPIC experience required

Skilled in placement for SNF / IPR / LTAC / Hospice / Home Health, DME, Behavioral Health needs, and difficult complex placements

Care management skills including assessment, care coordination, and discharge planning

Description :

Join our team as an RN - Case Manager.

This role requires a minimum of 3-5 years of experience in Case Management.

Candidates must have previously traveled as an RN-Case Manager.

The patient population will vary and may include adult medical / surgical, cardiac, orthopedic, and possibly pediatric patients.

Candidates should have a strong background in care management and placement coordination.

Experience with EPIC is a requirement for this position.

The department culture is very supportive and teamwork-oriented.

Managers and educators provide communication and support during changes.

The average patient census and caseload is typically at capacity, with 14-16 patients depending on unit assignment.

Travelers will work on holidays unless previously approved time off is granted.

Weekend rotation will depend on specific requisitions.

Support within the department includes a director, managers, a Chief Medical Officer, and a supportive UM team.

Peer case managers and social workers will be available to assist.
