Nurse Practitioner

Judge Healthcare is seeking a Nurse Practitioner for an opening with our biggest client!

Scope of Role : The Nurse Practitioner (NP) within the Senior Community Care (SCC) program plays a crucial role in coordinating and delivering clinical care to our highest-risk health plan members.

SCC is a longitudinal, integrated care delivery program that operates in a facility setting. The NP collaborates with an interdisciplinary team to provide comprehensive care. Responsibilities :

• Conduct Comprehensive Assessments :
• Perform thorough assessments of patients' health status, including physical, mental, and social aspects.
• Identify health risks and develop appropriate care plans.
• Manage Acute and Chronic Conditions :
• Effectively manage acute and chronic medical and behavioral conditions.
• Prescribe medications, order diagnostic tests, and interpret results.
• Accurate Documentation :
• Ensure accurate and complete documentation, including ICD-10 coding.
• Maintain electronic health records (EHRs) with attention to detail.
• Collaborate with the Interdisciplinary Team :
• Communicate and collaborate with other healthcare professionals, including physicians, nurses, social workers, and therapists.
• Participate in care conferences and case reviews.
• Advanced Illness and Care Planning :
• Engage in advanced illness conversations with patients and their families.
• Assist with advanced care planning, including end-of-life discussions.
• Patient Education and Counseling :
• Provide patients and caregivers with education on managing health conditions.
• Offer counseling related to lifestyle modifications, preventive measures, and treatment options.

Required Qualifications :

• Active and unrestricted license as a Nurse Practitioner in the state where you reside or the ability to obtain one by the start date.
• Graduation from an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program.
• Board certification through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) in one of the following specialties :
• Family Nurse Practitioner
• Adult Nurse Practitioner
• Gerontology Nurse Practitioner
• Adult-Gerontology Acute Care Nurse Practitioner
• Current, active DEA licensure / prescriptive authority or the ability to obtain it post-hire, per state regulations.
