About The Position

Job Title : RN Home Health

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty.

License : Must have an active state nursing license in hand.

Certifications : CPR, BLS

Must-Have : Proficiency in HomeCare HomeBase (HCHB) and Oasis.

Description :

Provides, coordinates, and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures.

Responsible for the competent application of the nursing process.

Reports to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Conducts home visits and participates in periodic office meetings.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average number of patients per day is between 5-8.

Each type of visit and activity (documentation, travel, etc.) holds a different point value.

Expected to achieve 30 productivity points per week.

May involve types of cases such as Start of Care, Wound Care, and Follow-up Visits.

Not applicable for first-timers; must have recent specialty experience.

Weekend work requirements may vary per location.

On-call requirements may also vary by location.

Must be willing to travel up to 75 miles from the home clinic.

Mileage will be reimbursed at the IRS standard rate.

Local candidates are accepted.

All documentation must be completed within 24 hours of the appointment with the patient.

Delayed or incomplete documentation that impacts billing and follow-up patient care will result in cancellation of the contract.

No guaranteed hours with this contract.

Clinician must hit weekly productivity points of 30, equating to approximately 40 hours per week.

All overtime must be pre-approved.

If not receiving enough patients each week, the clinician has the opportunity to be floated to another nearby clinic to pick up additional patients.

Must be open to traveling to patients' homes daily and to the clinic for weekly meetings.

Confirmed understanding of all job expectations is required.
