Job Description

About myPlace Health

myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.

Our mission is simple : to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.

myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.

About this Role

If you’re passionate about helping older adults live safely and comfortably at home, this role is for you. As a Home Care Coordinator (LVN) , you’ll play a vital role in connecting participants to the compassionate care they deserve—whether that’s through trusted home care partners or your own hands-on support.

You’ll work closely with our Interdisciplinary Team (IDT) to design thoughtful care plans, coordinate visits, and ensure every detail is captured in our EMR system. Each day brings new opportunities to make a meaningful difference and build lasting relationships that help our participants thrive in their communities.

Join us in creating a more connected, caring model of healthcare—one participant at a time.

What Does Success Look Like As Our Home Care LVN?

  • Coordinating Home Care Services - Collaborate with trusted home care agencies to ensure our participants receive personalized, compassionate care that meets their unique needs.
  • Contributing as a Key Team Member - Be an integral part of our Interdisciplinary Team (IDT), conducting home assessments, creating thoughtful care plans, and gathering vital information to guide the best possible services for our participants.
  • Providing Compassionate Nursing Care - Deliver dedicated nursing care within the LVN scope, whether in the clinic or in participants' homes, with a focus on comfort and well-being.
  • Documenting with Purpose - Maintain clear, accurate, and timely records in the Electronic Medical Record (EMR) system, ensuring seamless communication and continuity of care for all participants.
  • Collaborating for Participant Success - Work closely with home care employees to monitor participants’ health, ensuring any changes are promptly communicated to the RN and Physician for ongoing, quality care.
  • Supporting Smooth Transportation - Partner with the Transportation Coordinator to make sure participants can easily get to their appointments, removing any barriers to care.
  • Managing Appointments - Assist with scheduling home care visits and provide backup support for other appointment scheduling as needed, ensuring no gaps in care.
  • Ensuring Quality Care - Oversee visits from external agencies to ensure that all services provided meet the highest standards of quality and truly benefit the participants’ health and well-being.
  • Be a Team Member – Step in when necessary to assist the care team, fostering a positive and efficient atmosphere that benefits both colleagues and participants alike.

What does an ideal profile look like?

  • Experienced and Knowledgeable : You have a current LVN license and have experience in providing nursing care in both clinic and home care settings. Your understanding of the LVN scope of practice allows you to deliver high-quality, compassionate care to participants.
  • Person-Centered Approach : You’re deeply committed to tailoring care to each participant’s individual needs. You have a passion for enhancing the lives of those you serve, providing care with empathy, patience, and respect.
  • Team-Oriented : You work effectively as part of a multidisciplinary team, collaborating with physicians, nurses, and other healthcare professionals. You actively contribute to care plans, assessments, and communication, always with the goal of improving participant outcomes.
  • Excellent Communication Skills : You excel in both written and verbal communication. You document accurately in EMR systems, and you’re skilled at conveying important health information to team members and participants' families.
  • Adaptable and Flexible : You’re comfortable working in dynamic environments, whether in participants' homes, in the clinic, or in collaboration with outside agencies. You can adjust to varying needs and priorities with a positive attitude.
  • Organized and Efficient : You keep accurate records, stay on top of appointments, and manage your time efficiently to ensure care is delivered on schedule. Your attention to detail ensures that nothing falls through the cracks.
  • Compassionate and Caring : Your kindness and genuine care shine through in your interactions with participants. You go beyond just the physical aspects of care, offering emotional support and a sense of security to those you serve.
  • The Fine Print

  • You’ll be on the move : This role is up to 100% in the field , visiting participants in their homes and throughout the community to ensure they receive personalized, high-quality care.
  • Your home base : When you’re not out in the field, you’ll be at the center—collaborating with your teammates, coordinating care, and keeping communication flowing.
  • Your schedule : Monday through Friday, 8 : 00 a.m. to 5 : 00 p.m. , with some flexibility for earlier or later shifts to meet participant needs.
  • Team flexibility : While this is primarily a weekday role, occasional weekend support may be needed to ensure continuity of care.
  • A role that keeps you inspired : If you thrive on variety, teamwork, and making a real difference each day, this position offers purpose and connection in every moment.
  • What Does The Physical Environment Look Like?

  • Office Setting : Work is performed in a professional office environment within a healthcare facility, using computers, phones, and scheduling software.
  • Seated & Active Work : The role involves extended periods of sitting at a desk, typing, and speaking on the phone, along with occasional movement throughout the center to collaborate with staff.
  • On the Road : You’ll spend significant time traveling to participants’ homes and throughout the community, which may include exposure to natural weather conditions and long periods of sitting in traffic.
  • Home Visit Environments : Some participant homes may present unique conditions—such as limited space, clutter, or environmental challenges (e.g., hoarding or pest concerns). Safety is always our top priority, and we approach each visit with empathy, awareness, and professionalism to ensure both participant and team well-being.
  • Fast-Paced & Team-Oriented : You’ll work in a dynamic, collaborative environment, balancing multiple priorities while ensuring participants receive seamless, high-quality care.
  • Noise Level & Distractions : The office environment may have moderate noise due to phone calls, conversations, and daily activity in the center.
  • Technology Use : Regular use of computers, scheduling software, and EMR systems to document and coordinate participant care.
  • Safety & Infection Control : This role requires adherence to all safety, driving, and infection control protocols, including the use of personal protective equipment (PPE) during home or clinic visits to ensure the safety of both participants and team members.
  • What’s In It for You?

At myPlace Health, we believe that when we take exceptional care of our people, they can take exceptional care of our participants. That’s why we’ve built a comprehensive, people-first rewards package designed to support your well-being—professionally, financially, and personally.
