Position description

The Department of Family and Community Medicine (DFCM) at the University of California Davis (UCD) School of Medicine envisions a healthier and more equitable world.

In service to this vision, the mission of the Department is to advance the field of family medicine through innovative education, person-centered care, research and other forms of scholarship, community engagement, and advocacy, to improve population health and equity.

To help advance this mission, the DFCM at the UCD School of Medicine is recruiting for a highly skilled, engaging, and visionary full time faculty member and leader in the Professor of Clinical X or Health Sciences Clinical Professor series at the Associate or Full Professor rank, to serve as the Department's inaugural Vice Chair for Clinical Affairs.

This position reports to the Department Chair and provides Department level leadership across all our educational programs and represents the DFCM in the UC Davis (UCD) School of Medicine and in the leadership teams of community-based organizations we partner with in care and training.

Faculty in the Clinical X series is expected to engage in professional competence and activity, teaching, research and creative work, and University / public service.

Faculty in the Health Sciences Clinical Professor series are expected to engage in teaching, professional competence and activity, scholarly or creative activities, and University / public service.

The DFCM is located on the UCD Health campus in Sacramento, California, and headquartered at the UCD School of Medicine, which US News and World Report ranked #3 nationally for student diversity, #7 for family medicine, and #9 for primary care in 2023.

Under the leadership of Department Chair Anthony Jerant, MD, the DFCM is in an and exciting phase of its 50-year history at a time of rapid transformation in healthcare.

Known for outstanding education, clinical care, and ground-breaking research, the Department has over the past five years greatly expanded its presence in the local community, forging several mutually beneficial and successful clinical training relationships with partner organizations.

These partnerships are designed both to ensure excellent training of our students and residents in caring for underserved and vulnerable patients most in need of care, and to improve the health of individuals, families, and communities locally, regionally, and beyond.

We are in Sacramento, the capitol of trend-setting California - a highly diverse and growing community (metropolitan area population 2.

4 million), and a great place to live and work. The city and its surrounding communities are booming with many cultural activities, world-renown farm to table dining, and a plethora of outdoor activities to enjoy.

The Department includes 23 faculty physicians, 5 staff physicians, and 42 resident physicians. We are a diverse group of clinicians, educators, and scholars who strive to deliver outstanding care to all our patients, improve the way primary care is provided, and educate the next generation of clinicians including family physicians, all pursued through an equity lens.

The Departmental commitment to equity in care access, delivery, and health is exemplified by our inpatient and outpatient clinical and training sites, all of which have a strong focus on providing high quality, evidence-based, person-centered care to medically under-served and vulnerable patients, families, and communities.

Our inpatient care sites include the UCD Medical Center hospital, part of a complex academic health system that among other missions serves as the regional safety net for those most in need, and at nearby Mather Veterans Administration Hospital (MVAH)

Our outpatient in person and telemedicine care is delivered at four sites located on or close to the UCD Health campus :

At the Ellison ACC Family Medicine Clinic practice, located on the UCD Health campus, currently there are 6.8 full time equivalent (FTE) faculty and staff physicians providing care and precepting, and 29 residents with continuity panels, collectively seeing approximately 2000 patients per month.

This is an Accreditation Council for Graduate Medical Education (ACGME) approved continuity FM training site.

At the One Community Health (OCH) FQHC located in nearby Midtown, Sacramento, there are 1.7 FTE staff and faculty DFCM physicians providing care and resident precepting, and 13 residents with continuity panels.

This is ACGME approved continuity FM training site.

  • At CommuniCare FQHC offices located in nearby West Sacramento, Davis, and Woodland, there are 0.7 FTE staff and faculty DFCM physicians providing care and precepting, and 16 residents providing longitudinal continuity care.
  • At the Sacramento County Primary Care Center (PCC) FQHC located in the Oak Park neighborhood of Sacramento, adjacent to the UCD Health campus, there are a combined 1.

5 FTE staff and faculty DFCM physicians providing care and precepting, and 13 residents providing longitudinal continuity care.

EXPECTATIONS

Reporting structure : The Founding Vice Chair of Clinical Affairs reports to the Department Chair. Direct faculty reports to the Vice Chair are :

o Medical Director, Ellison ACC Family Medicine Clinic

o Inpatient Care Leads (2) : Medicine / Pediatrics and Obstetrics and Infant Care

o The DFCM Faculty Leads (one per site) from our four community partner clinical / training sites : the One Community Health, CommuniCare, and Sacramento County Primary Care Center FQCHs, and the Mather VA Hospital

Responsibilities : The Founding Vice Chair of Clinical Affairs and Community Health will be recognized as an institutional leader by UCD Health, UCD School of Medicine, and our partner community organizations in relation to equitable primary care access, delivery, and outcomes.

Leadership and administrative responsibilities (50% effort)

  • Oversee clinical programs, quality, and safety improvement efforts, and support medical directors and other leaders, across all DFCM clinical settings.
  • Plan, manage, implement, and evaluate clinical programs that promote access to and delivery of value-based care in a cost-efficient, equitable manner.
  • Continue and expand efforts to redesign practice to achieve optimal patient outcomes and improve the clinical team's fulfillment in practicing family medicine.
  • As a member of the department's executive leadership team, contribute to the development and implementation of departmental strategic planning, budgeting, management, and operations, including developing projections, maximizing clinical productivity, and managing revenue and expenditures.
  • Meet regularly with clinical operations leaders from UCD Health and from our community partner organizations, to help align DFCM's strategic business vision and business development with the goals of the health system and, to the extent possible, those of our community partners.
  • Serve on health system and committees and represent the DFCM by advocating for patients and clinicians and working collaboratively to develop solutions to problems.
  • Collaborate with the Chair, Department and practice administrators, and UCD Health Public Affairs and Marketing to develop and maintain patient outreach initiatives to market and enhance the array of services offered by the DFCM.
  • Participate in or lead efforts to recruit, retain, and develop excellent clinical faculty and staff to care for patients and precept our residents at our various clinical and training sites.
  • Oversee clinical credentialing, onboarding, and proctoring of clinical faculty across the DFCM with support from designated Departmental and partner site administrative staff
  • An especially critical role of the Vice Chair is to develop and maintain close working relationships with the DFCM faculty site leaders and the clinical and administrative leaders and managers at each of our clinical and clinical training sites to develop strategically aligned goals to increase productivity;
  • enhance care quality and patient and provider experience metrics; anticipate and respond to new and evolving (e.g., value-based) payment models;

improve the health of underserved and vulnerable patients, families, and communities; and increase our engagement with and accountability to the communities we serve, while ensuring that trainees at each site participate in and learn from these efforts.

Another critical role of the Vice Chair is to partner with the various site leaders and their teams to identify, advocate for, and oversee shared, uniform adoption of clinical and clinical training "best practices" across the sites.

Through our collective work at these diverse clinical and training sites, led or facilitated by the Vice Chair, the Department aims to enhance the quality of life for underserved and vulnerable patients, families, and communities, while educating future generations of highly skilled, compassionate, service-oriented, and personally fulfilled family physicians well-equipped to employ the state-of-the-art approaches they learned in

Clinical responsibilities (40% effort)

Build and provide top quality primary care to an outpatient panel of size proportionate to clinical effort.

Teaching and scholarship responsibilities (10% effort)

Provide clinical ("bedside") and didactic teaching to residents, medical students, and potentially other trainees (e.g.

nursing students), with a particular focus on practice and panel management, population health, billing and coding, health care financing, health insurance, disparities in healthcare access and delivery, and other related topics.

Promote, support, and personally participate in research and / or other forms of scholarship (e.g., evaluation, curriculum), again with a particular focus on "delivery and practice of healthcare" topics.

  • Current M.D. or D.O. medical license
  • Active board certification in Family Medicine
  • Medical licensure or eligible for licensure in the state of California
  • Track record of progressive success in the leadership and administration of large clinical programs in complex adaptive systems - preferably in the academic healthcare environment and encompassing both inpatient and outpatient care delivery as well as partnerships with community-based primary care organizations
  • Demonstrated experience and success in family medicine clinical programs, education, and research or scholarly work
  • Demonstrated experience and success working in and fostering a diverse faculty, staff, and learner environment
  • Eligible for appointment as Associate professor or Professor at the UC Davis School of Medicine
