Case Manager Pooled Trust
The Pooled Trust Case Manager is responsible for supporting people with disabilities and their families accessing and navigating their pooled special needs trust, and governmental benefits and linkage to community resources. The Pooled Trust Case Manager will primarily interact on the phone and through email with individuals, families, care managers, attorneys, and other interested parties associated with the Trust. This full-time position requires strong communication, people skills, and organizational skills.
Trust Services
- Communicate with individuals, families, and attorneys to assess the current situation of beneficiaries.
- Maintain knowledge of Special Needs Trust Rules and Regulations and information from Medicaid and Social Security Administration.
- Maintain an understand public benefits and the potential impact of distributions on those benefits.
- Process disbursement requests in compliance with public / government benefit criteria.
- Conduct presentations and webinars on the Pooled Trust and other relevant topics.
- Work with Trust team to update Policies & Procedures, Handbooks, Beneficiary Training Materials, FAQ's, etc.
Care Management Support
Communicate and develop relationships with beneficiaries, guardians, family members, care managers and attorney(s) to meet the beneficiary's individual needs.Develop and monitor beneficiary budgets.Develop and monitor care management plans.Provide linkage and referral to beneficiaries and their representatives with care management and community services.Monitor and manage the care management services to ensure service delivery.Conduct home and community visits to monitor beneficiary services and assess needs, as needed.Communicate with other team members regarding beneficiaries and their needs.Public Benefits
Assist Beneficiaries and their representatives to complete public benefit applications.Provide regular reports to keep public benefits eligibility and respond to requests from these programs.Education
Minimum Bachelor's DegreeDegree in Social Work or Human Services related field preferredExperience
Knowledge of disability benefits (Social Security, Medicare, Medicaid, etc.)Minimum of 3 years' experience in social services industryAbility to effectively communicate orally and in writingAbility to work collaboratively with internal and external stakeholdersAbility to handle confidential information in a discreet, professional mannerDetailed and accuracy of processing materialsProblem solvingRecording keepingProblem Solving / Analysis.Customer / Client Focus.Results Driven.Flexibility.Extensive computer use.Must be proficient with Outlook, Teams, Excel, Word, and CRM databasesModerately quiet officeThis job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.This role requires some field engagement, with regular visits to beneficiaries, community locations, and other relevant sites to engage directly with participants and stakeholders. Participation in state and / or national conferences.
