About The Position

Job Title : Registered Nurse

• Experience in care coordination and discharge planning
• Knowledge of CPT codes, Department of Health regulations, and HEDIS measures
• Strong background in case management is preferred

Description :

Candidates must be able to start immediately.

The position requires face-to-face interactions within designated areas.

Case managers will make calls to enrollees to complete comprehensive assessments.

They will develop individualized care plans that incorporate social determinants of health.

Collaboration with healthcare providers is essential to ensure successful care plan execution.

Successful candidates must be computer proficient and familiar with software for telecommunication.
