We are seeking to fill this LCSW position with someone who is self-directed, flexible, and enthusiastic. Since this is a new team, the ideal candidate will be autonomous and creative, able to contribute to the development of the role based on identified nephrology patient needs.
The ideal candidate will have the qualifications necessary to assist our MSW with case management in the nephrology clinic and hospital, providing feedback and support, as needed (estimated 40% effort).
Additionally, the LSCW will provide support to patients receiving patient education, working collaboratively with an interdisciplinary team to provide psychosocial support, to promote self-management behaviors, and to meet the goal of early selection of treatment modality for patients with a high risk for end stage renal disease (estimated 50% effort).
