• This role is primarily remote with 1-2 days per week of in person work at our Petaluma Clinic. Must reside in CA to qualify for this role.

Summary : The NP / PA Clinical Support Team Provider will serve as an integral member of the Petaluma Health Center’s primary care team and offer critical treatment plan follow up and support for patients at Petaluma Health Center’s sites. The Clinical Support Team Provider will ensure that patients, particularly those at high risk or with complex care needs, have access to timely and appropriate care between visits with their primary care providers. Critical to the role will be the ability to navigate the electronic health record (EHR) and manage the flow of information to improve health outcomes for patients.

Language Skills : Ability to effectively present information and respond to questions and requests from patients, co-workers, and others as necessary. Bilingual in English and Spanish highly preferred.

Salary Scale : $120,000 to $173,806 per year

The compensation amounts listed above do not include relocation assistance, sign-on bonus, productivity or quality / service incentives, call pay, extra shift incentives, if applicable to the position.
