Nurse Practitioner NP

RiteMed Group, a provider-focused organization specializing in Nurse Practitioner staffing in long-term care, is seeking a dedicated Nurse Practitioner for Complete Care at Milford Manor, a skilled nursing facility. This is a full-time role with a supportive environment and the option for salaried or per-note compensation.

Key Responsibilities

Provide on-site primary care for residents in both long-term and subacute units.

Perform comprehensive assessments, manage chronic conditions, and treat acute issues.

Collaborate with nursing staff, therapy, and the interdisciplinary care team.

Document visits thoroughly in EMR systems.

Help reduce avoidable hospitalizations and improve resident outcomes.

Qualifications

Active Nurse Practitioner license in New Jersey.

Minimum of 6 months NP experience; ICU or hospitalist background a plus.

Strong clinical judgment and communication skills.

Comfortable managing complex patient populations in a skilled nursing setting.
