Independent Living Coach - Group

Independent Living Group Leader is responsible for implementing assigned participants" Person Centered Plans (PCP) working with participants and their families to improve socialization, self-help, communication and pre-vocational skills. The Independent Living Group Leader will work with assigned participants at The League and in the community.

Essential Duties and Responsibilities :

• Communicate with participants, families, professional staff and community members in a person-centered manner
• Develop a positive working relationship with assigned participants and families / caregivers
• Provide community-based developmental vocational habilitation services consistent with participants' PCPs
• Develop good working knowledge of participant's Behavior Plan, if applicable, utilizing strategies and techniques within the plan to assist participant with decreasing target behaviors / increasing communication skills
• Implement skill acquisition in areas of , daily living skills, socialization, leisure, basic academics, motor skills, communication, and self-advocacy
• Participate in the development, implementation, monitoring and updating of assigned participants' PCPs
• Assist assigned participant with toileting and personal hygiene as needed
• Maintain complete and accurate participant dailydata entry in iCare, tracking behavior data, if applicable, and in other required documentation
