Job descriptionConduct client assessments and evaluations including screening for co-occurring disorders. Develop and implement treatment plans for clients that address chemical dependency and co-occurring disorders. Develop discharge plans with clients that include provisions for co-occurring disorders if identified. Monitor client fee payments and refer clients for financial staffings. Retrieve drug screen results and document in file. Provide case management services for clients. Provide individual, group and family counseling, as necessary. Prepare and maintain clinical records within required time frame. Submit reports as required. Participate in clinical staffing and team meetings. Participate in weekly therapeutic community meetings. Make referrals to appropriate resource agencies and document outcomes of referrals. Provide crisis intervention, when needed. Assist in training and education of other staff members, interns and students. Maintain client rights as defined by DCF. Maintain the highest standards in ethical conduct and confidentiality. Maintain clinical records in accordance with Agency standards. Follow all established Agency policies and procedures. Model ideals of integrity, loyalty, and teamwork. Sustain a positive, caring, supportive recovery-oriented environment Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Knowledge of 12-step program, behavior modification, case management and addiction. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position. Regularly required to sit and talk or hear. Frequently required to stand and / or walk; use hands to finger, handle or feel objects, tools or controls; and reach with hands and arms. Specific vision requirements include close vision, distance vision, peripheral vision and the ability to adjust focus.

Primary function
- Provide clinical intervention and case management for clients diagnosed with chemical dependency or co-occurring chemical dependency and mental health disorders by performing the following duties.
