Take Your Therapist Home

As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program!

This role is focused on Home Health Care in the Ann Arbor and surrounding areas with a steady referral source from multiple skilled nursing facilities. This role includes PTA supervision and provides the opportunity to pick up additional time at nearby SNF

In a world of shorter and shorter SNF stays, Take Your Therapist Home (TYTH) is an unique program where clinicians work directly with patients in the SNF and home health providing a continuity of care allowing for you to have a greater impact on patient outcomes!

Our perspective is unique in providing continuity of care across the health care continuum in the Skilled Nursing, Home Health, and Outpatient settings.

Facility Location : Ann Arbor 4701 E. Huron River Drive Ann Arbor, MI 48105. This is a home care role providing Home Health therapy services with the option to have additional time in the SNF. Home Health geographic area is the vicinity around Ann Arbor.

Schedule : Full Time, Monday-Friday or Tuesday-Saturday (Tues-Sat schedule will receive premium shift pay).
