Job Description :
The RN Home Services Case Manager utilizes clinical expertise and critical thinking skills to direct resource utilization to develop and implement a plan of care that is both cost effective and is designed to assist the patient in moving to functional independence and improve quality of life. The RN Home Services Case Manager is the primary caregiver upon admission, recertification, and discharge for the purpose of assessing progress toward stated goals. This role is responsible for the completion of the Outcome and Assessment Information Set (OASIS) and tracking progress against admission assessment. The incumbent has accountability for the coordination of caregivers and supplies necessary in the provision of patient care. This position works collaboratively with patients, patient families / significant others, healthcare providers, insurers, community resources, and other involved parties.

Minimum Qualifications :
Current Registered Nurse (RN) license in state of practice. Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.
Bachelor of Science in nursing (BSN) from an accredited institution (degree verification required).
RNs must obtain their BSNs within four (4) years of hire or promotion date. If there is an existing education agreement, that agreement will take precedence.
Basic Life Support Certification (BLS) for Healthcare Providers.
Current driver’s license, insurance, reliable transportation, and an acceptable driving record.
Unit specific certification as required (i.e., ACLS, PALS, etc.).
Demonstrated clinical nursing experience.
Demonstrated knowledge of home health or hospice.
Ability to communicate effectively both verbally and in writing.
Preferred Qualifications :
BSN from an accredited institution (degree will be verified)
Case Management Certification
One (1) year experience working in home health or hospice.
Three (3) years of clinical nursing experience.
Experience working with third-party payors
Previous experience in case management, utilization review, and / or discharge planning.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$40.39 - $60.96
