An opportunity is available for a skilled and dedicated Home Health Registered Nurse (RN) to provide comprehensive nursing care directly to patients in their residences. This role involves delivering patient-centered care with a focus on clinical precision and effective communication while working independently in a field-based setting.\n\nResponsibilities include conducting thorough patient assessments using the OASIS tool, developing individualized care plans, and administering skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management. The RN will monitor patient progress, collaborate with interdisciplinary healthcare teams, and provide education to patients and families to enhance treatment adherence and preventive care. Accurate and timely documentation using the Home Care Home Base (HCHB) electronic system is essential, alongside adherence to all regulatory and agency standards.\n\nKey Qualifications and Experience : \n

• Active RN license in good standing within the state\n
• Minimum of one year recent home health nursing experience, preferably in a field-based role\n
• Proficiency in Home Care Home Base (HCHB) and OASIS assessments\n
• Strong clinical assessment capabilities and critical thinking skills\n
• Valid drivers license, reliable transportation, and auto insurance\n
• Excellent communication, organization, and time management skills\nPreferred : \n
• Bachelor of Science in Nursing (BSN)\n
• Experience managing 2532 points per week caseload\n
• Knowledge of Medicare and home health compliance standards\n\nJob Details : \n

• Schedule : Monday to Friday, 8 : 00 AM 5 : 00 PM\n
• Caseload : 2532 points per week (approximately 35 patient visits daily)\n
