This position requires expertise in OASIS documentation and proficiency with Homecare Homebase software, offering an excellent opportunity to provide care within a welcoming healthcare environment.\n\nKey Responsibilities : \n

• Conduct patient assessments and document information according to OASIS standards\n
• Develop and oversee individualized care plans\n
• Deliver direct nursing care, including wound care and medication management\n
• Work with multidisciplinary teams to promote optimal patient health\n
• Educate patients and families on health practices and self-care\n\nQualifications : \n
• Active RN license in Minnesota\n
• OASIS proficiency and experience with Homecare Homebase required\n
• 2 years of Home Health experience preferred\n
• Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission\n
