Position Status : Full Time

FLSA Status : Non-Exempt

Pay & Benefits : The pay range for this role is generally$49,300 - $57,000 annually (plus a sign on, 3 month, and 6 month bonuses in the first year of employment), commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY :

The Care Coordination Program focuses on prompt linkage to HIV care, ongoing retention in care, optimal treatment adherence, viral load suppression and reduced HIV-related mortality. The programs in this department work with people living with HIV / AIDS to improve overall health and establish self-management through case management, peer navigation, and health education services.

The Supportive Counselors will play a dynamic role in enhancing these services by providing direct support and counseling services to clients by conducting assessments, developing individualized care plans, and providing ongoing support.

DUTIES & RESPONSIBILITIES :

Conduct comprehensive assessments and intakes to evaluate for client eligibility and goals

Develop individualized care plans to help clients stay engaged in their own care

Provide one-on-one supportive counseling sessions

Promote stability and support by addressing goals as described in the service plan

Conduct group supportive counseling to address service plan goals and enhance mutual support between clients

Participate in outreach activities to engage new clients and inform the community of available services

Collaborate with other CHASI programs and external providers to make culturally appropriate linkages and referrals to help achieve client goals; perform follow up to ensure a “warm handoff”

Complete all progress notes and other reporting as required by internal practices and funders in a timely manner

Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values

Requirements

QUALIFICATIONS :

Master’s degree in mental health counseling, social work, or other related degreerequired

At least 2 years of experience providing case management or counseling services to people living with HIV / AIDS or other chronic illness or related population

Demonstrated experience with and / or understanding of providing evidence-based, client-centered, trauma-informed, culturally appropriate interventions rooted in harm reduction principals
