Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. • Location : LaGrange, KY, and surrounding areas\n
• Company : Sunbelt Staffing, in partnership with a nationwide home health agency\n
• Contract Length : 13 Weeks (with possible extension upon successful start and completion)\n
• Hours : Full-time, Scheduled 40 hours per week - 32 hours guaranteed\n
• Schedule : Monday-Friday, 8 am-5 pm, with occasional weekends and on-call duty (typically one weekend per month on-call)\n
• Productivity : 25 per week, 30-35 patients per week\n
• Compensation : Negotiable, based on experience and territory budget; paid hourly - not by visit; blended pay packages available for travelers; weekly pay\n
• Mileage Reimbursement : 0.48c per mile visit to visit, 150-250 miles per week expected\n

• Conduct patient assessments using OASIS, ensuring accurate and timely documentation.\n
• Develop and implement personalized care plans in collaboration with physicians and other healthcare professionals.\n
• Provide skilled nursing care, including wound care, medication administration, and monitoring of vital signs.\n
• Educate patients and their families on care plans, medication management, and other health-related topics.\n
• Maintain accurate patient records in Homecare Homebase (HCHB) and ensure compliance with agency standards.\n
• Travel to patient homes within thearea, utilizing reliable transportation.\n
• Participate in case conferences and team meetings to ensure coordinated and comprehensive care.\n\nRequirements\n
• Active Registered Nurse (RN) license in the state of Kentucky.\n
• Minimum of 1 year of home health experience working with adult and geriatric patients.\n
• Proficiency in OASIS and Homecare Homebase (HCHB) required.\n
• Oasis Certified strongly preferred.\n
• Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.\n
• Reliable transportation with a valid driver's license.\n
