Job Summary :

As a Care Coordinator, you will be responsible for :

  • Developing, monitoring, and evaluating home health care plans
  • Scheduling agency aides and ensuring shifts are covered, even on short notice
  • Managing a caseload of 100+ patients with a strong emphasis on retention
  • Matching clients with the most appropriate caregivers based on their needs
  • Communicating with referral sources and case managers to ensure excellent service
  • Handling caregiver and client concerns with professionalism and care
  • Reviewing care plans and addressing overutilization or staffing issues
  • Contributing to team goals and company mission

    • Qualifications :

  • High school diploma or equivalent (required)
  • Minimum of 2 years' experience in Home Care or Service Coordination
  • Experience using HHA Exchange and EVV systems (preferred)
  • Proven ability to manage confidential information
  • Strong communication and customer service skills
  • Proficiency in scheduling and computer applications
  • Comfortable with closing and lead follow-ups
  • Highly organized and detail-oriented
