FTE Status : Full Time and Part-Time opportunities available

Work Schedule : Day & PM shifts, with an every other weekend rotation and holiday rotation

Unit Description : the Transitional Care Unit at Emplify Health by Bellin is a specialized 4-bed unit dedicated to providing comprehensive care for patients transitioning from acute hospital setting to home or other care facilities. Our multidisciplinary team delivers personalized, patient-centered care aimed at optimizing recovery, enhancing functional independence and ensuring a safe and seamless transition to the next level of care.

Job Description :

Responsible for patient care under the direct supervision of a Registered Nurse.

Qualifications :

Listed on state of Wisconsin Nursing Assistant Registry required.
