About the Role
We are seeking an experienced Director of Case Management to lead and oversee hospital-wide Care Management, Utilization Review, Care Coordination, and Social Work programs. This is a key leadership position responsible for driving quality outcomes, resource utilization, discharge planning, and ensuring safe transitions of care across the continuum.
This role requires an executive-level healthcare leader with strong operational expertise, excellent communication, a history of results, and the ability to manage interdisciplinary teams in a fast-paced acute care environment.
Key Responsibilities
Departmental Leadership & Strategy
- Direct, oversee, and evaluate daily Case Management & Social Work operations.
- Lead the care coordination model, staffing, onboarding, training, and competency programs.
- Allocate resources to ensure top-tier productivity, documentation accuracy, and regulatory compliance.
- Ensure adherence to discharge planning standards, timeliness, and quality documentation.
Operational Excellence
Oversee core Case Management functions, including utilization management, LOS optimization, and care transitions.Analyze operational data and lead performance improvement initiatives.Collaborate with executive leaders including CNO, CMO, CFO, Quality, Compliance, and Ancillary departments.Drive compliance with hospital policies, SOPs, regulatory standards, and audit requirements.Team Leadership
Lead a team of :2 Direct Reports17 Indirect ReportsPromote team building, trust, accountability, and collaboration.Mentor teams in clinical resource utilization and complex discharge planning.Additional Duties
Represent the department in executive meetings.Manage complex patient reviews and escalations.Uphold organizational Mission, Values, and Code of Conduct.QualificationsRequired :
3+ years recent acute care Hospital Case Management experience (within the last year).2+ years Case Management leadership experience.Bachelor's Degree required.Active RN, LCSW, or LMSW license.Strong communication, executive presence, and ability to drive outcomes.Preferred :
Master's in Nursing, Business Administration, Health Administration, or related field.Position Details
