Job Description
The Enhanced Care Management (ECM) RN will be responsible for coordinating the care of patients enrolled in Chaparral's ECM program. The ECM RN is also responsible for Clinical Oversight of ECM Lead Care Managers to address the patient's medical and behavioral health needs. They will be active members of the Interdisciplinary Care Team meetings to discuss medication management / adherence, ensure appropriate follow up with Primary Care Provider, Behavioral Health Provider, and other specialist(s). The CM Nurse ensures that ECM members are receiving needed services and measuring progress towards the goals outlined in their patient-centered care plan.

Compensation : $45-60 / hr. Variable based on qualifications, experience, and other interview factors.
Physical Demands : Most work will be in the field working with CalAIM populations (people experiencing homelessness, adults with SMI / SUD, adults transitioning from incarceration, adults with complex medical needs). Will likely require substantial walking, standing, sitting in a car, and driving in various weather conditions.
