Job descriptionSelf-awareness —Self-awareness is a key component of person-centered planning; staff must be cognizant of their own cultural assumptions, psychological development and temperament, personality dynamics, and prejudices to avoid imposing their beliefs on the process. Personal integrity —In the context of person-centered planning, personal integrity means having minimal conflicts, clear values of caring for the person, acting in accordance with those values, and acting consistently over time. Informed decision making —The ability to help the person understand what the options are and to support the exploration of potential options in order to enhance decisions. Yielding control —The ability to have planning driven by the person through self-direction and self-determination, including supporting consumers to initiate planning. Team building —Building the plan is a collaborative task, and the staff fosters a team environment. Team building keeps the group of individuals around the person in a Strengths-based thinking —Focus is on the positive attributes of a person, the process is person-led, and centered on strengths-based outcomes and positive attributes. Active and reflective listening —Facilitators use active listening throughout the planning process, giving their full attention, using body language and responses that demonstrate their consideration and understanding of what the speaker is communicating. The goal of active listening is to ensure the speaker feels heard and understood. Staff use reflective listening, paying special attention to the content, feelings, and meaning behind the message and reflecting back to the speaker to demonstrate understanding and clarify the message. The purpose of reflective listening is to act as a mirror or reflection of the speaker to optimize their communication. Understanding of living best life —Living one’s best life is subjective and involves reaching targeted levels of growth, leading to taking actions that help the person discover and hone their interests, talents, and passions. The facilitator supports the person to articulate what his or her best life looks like, and to tailor their plan to fit their ideal. Minimum Associate of Arts Degree and / or some college preferred. MUST have a high school diploma or GED. Have and maintain current Red Cross First Aid Certificate and CPR Certificate. Fluent in speaking, reading, and writing in English; Spanish, preferred. Minimum of 5 years of experience preferred. Monday – Friday 8 : 00am – 4 : 30pm. Some weekends or evening as needed for special events / community events. 403B available with employer 4% provided after employee puts in 6%. Health, Vision, Dental benefits available. BASIC RESPONSIBILITY :
Under the direction of the Director of Programs the Program Innovation Leader will be responsible for the daily operation of their assigned program. The Program Innovation Leader reports to the Director of Programs.
SPECIFIC DUTIES :
1. Maintain current accurate records of program activities.
2. Conduct meetings with participants as required, but not limited to annual meetings.
3. Prepare and complete written reports on participant progress, meetings, daily logs, attendance logs, and incidents.
4. Responsible for following safety procedures and for reporting all incidents.
5. Responsible for contacting parents and regional centers for any participant issues. Documentation of contact is necessary and required.
6. Assist / train the Direct Care Providers to understand their role, expectations and responsibilities in the program.
7. Plan daily activities that follow a person-centered approach and support the participant’s individual program goals.
8. Encourage participants to join activities while promoting individual choice and decision-making opportunities.
9. Attend departmental staff meetings and trainings.
10. Perform additional tasks and duties as may be assigned from time to time.
QUALIFICATIONS : General :
- Must be at least 21 years of age.
- Must pass a criminal background check.
- Must have a current California driver’s license with a good driving record.
- Knowledgeable of Microsoft Office Suite, Powerpoint and Excel.
- Can lift a minimum of 45lbs.
- Strong communication, teambuilding and leadership skills
- Willingness to learn and be flexible
- Ability to act with integrity, professionalism, and confidentiality.
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
