Routine Caregivers are responsible for providing services in the consumers’ homes based on services documented in consumers’ care plan.
This position is hands on with providing support to consumers in the following areas but not limited to personal care, homemaker, respite, and or companionship.
Routine Caregivers are part of the Care Coordination Department team. This team includes Consumer Care Coordinators, Consumer Care Supervisor, and Senior Consumer Care Manager.
The Care Coordination Department is responsible for identifying and assigning Routine Caregivers that meet the consumer’s service needs.

Duties include, but are not limited to the following :
- Perform tasks as described on consumer care plan in the following areas but not limited to personal care, homemaker, respite, and / or companionship
- Drive to consumers’ homes to provide services in the consumers’ homes only
- Accompany consumers’ to approved appointments in third party transportation only
- Utilize agency application to clock in and out for completed shifts with consumers
- Document in agency application mileage according to agency process
- Verbally report and submit written report for all incidents and / or accidents
- Responsibilities include, but are not limited to the following :
- Read consumer care plans to assure understanding of the consumer’s needs
- Plan your drive to consumer homes to assure on time arrival
- Manage your shift time to assure clocking out on time
- Communicate with Consumer Care Coordinators for any questions and / or concerns regarding assigned consumers
- Build and maintain rapport with consumers and their circle of supports
- Provide services in a compassionate and caring demeanor
- Exercise professional boundaries with consumers and their circle of supports
- Monitor and report any changes in consumers’ conditions
- Maintain a healthy and comfortable environment in the consumer’s home for the consumer and yourself
Complete paid training on a yearly basis (virtual / in person)
- Implement mandating reporting
- Communicate changes in your availably in a timely manner to lessen the impact on consumer services
Maintain consumer confidentiality
Implement critical thinking to work effectively in a work environment with potential high levels of interruptions
