Job Overview:

We are currently seeking a dedicated and experienced Physical Therapist (PT) to join our team for a 13-14-week contract assignment located in Gresham, OR. This position offers a great opportunity for a compassionate and motivated PT who is passionate about helping aging adults regain mobility and independence.

Key Responsibilities:

• Evaluate patients’ physical conditions and develop individualized treatment plans
• Provide direct physical therapy services to patients in accordance with the plan of care
• Collaborate with a multidisciplinary team including OTs, PTAs, and nursing staff
• Monitor patient progress and adjust therapy plans as needed
• Document all patient interactions, progress notes, and outcomes in a timely manner
• Educate patients and families on at-home exercises and post-discharge care
• Maintain compliance with facility policies, federal and state regulations, and professional standards

Requirements

Qualifications:

• Active and valid Oregon Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Strong clinical reasoning and decision-making skills
• Ability to work independently and as part of a care team
• Excellent interpersonal and communication skills
• Familiarity with electronic documentation systems (EMR)
