Job description

PURPOSE : Behavioral Health Department offers both addiction and mental health services and is committed to providing successful treatment and ongoing recovery to our patients. We focus on developing an individualized recovery plan for each participant. Both individual and group sessions are utilized in our program.

The CWWCHC Medication Assisted Treatment (MAT) program is the use of medications combined with counseling and behavioral therapy for patients diagnosed with substance use disorders. The MAT program is primarily for the treatment of addiction to opioids such as heroin and prescription pain relievers that contain opiates.

DUTIES & RESPONSIBILITIES :

  • Researching and advocating for appropriate assistance resources or services for patients.
  • Assist patients by helping them cope and manage stress, depression and / or anxiety.
  • Maintain Patient records in a timely manner (completion within 24 hours).
  • Ensure patients are scheduled and attend behavioral health visits.
  • Be the point of contact for Medication Assisted Treatment (MAT) coordination.
  • Track and document coordination of care activities to ensure proper notification, conflict resolution, and appropriate follow-through by providers.
  • Ensure exceptional service and professionalism, including problem resolution.
  • If licensed, diagnose patients with mental health conditions, emotional or addictive disorders and provide counseling services and other resources to individuals, family members or small groups, that promote healthy coping strategies and improve overall well-being.
  • Communicating with patients' care teams.
  • Providing crisis intervention as needed.
  • Referring individuals to appropriate treatment centers, as indicated.
  • Provides care coordination / care management and education to patients and staff members as required.
  • Maintain strict confidentiality.
  • Be punctual and maintain regular, reliable attendance.
  • Perform other duties as directed.

    Duties and Responsibilities :

    EDUCATION / WORK EXPERIENCE :

  • Master of Social Work and Licensed Clinical Specialist Addiction or Licensed Clinical Social Worker preferred.
  • The ability to work independently and handle multiple priorities and tasks.
  • The ability to work effectively with people of varying degrees of education and professional expertise in a multi-disciplinary team setting.
  • Clinical background and 3 years of nonprofit experience is desirable.
  • CONFIDENTIAL MATTERS :

  • All patient care and program activities are confidential. Violation of confidential matters is grounds for immediate dismissal.
  • SKILLS AND QUALIFICATIONS :

  • Experience in a medical office / Federally Qualified Health Center setting.
  • Understand and promote mission, vision, and values of CWWCHC both in the workplace and in the community.
  • Excellent organizational, leadership, and communication skills written and verbal.
  • Experience working within a behavioral health and / or MAT Program is desired.
  • Must be sensitive to the needs and situation of multi-cultural population from a variety of income levels.
  • Proficient with Microsoft Word, PowerPoint, Excel, and Outlook.
  • Working knowledge of social theories and practices.
  • Social perceptiveness and empathy.
  • Ability to build and maintain professional helping relationships.
  • Ability to relate and communicate with diverse populations and groups.
  • Ability to make psychosocial assessments and develop and implement viable care plans.
  • Excellent listening and communication skills.
  • Experience with intimate partner violence counseling highly desired.
  • Experience with electronic health records (EHR).
  • Athena EHR software experience highly desired.
  • Ability to provide after-hours service as needed.
  • Bilingual preferred.
  • Must possess a working knowledge of the current Diagnostic and Statistical manual of Mental Disorders (DSM).
  • Must be able to use protocols, guidelines, and benchmarks to implement policies and develop quality improvements while utilizing best practices.
  • PHYSICAL DEMANDS : Those inherent to working in the community requires one to be in good health - able to do lots of

    walking, talking and standing for long periods.

    EFFECTS OF ERRORS : Errors may expose the Center to Liability and Embarrassment.

    HOURS OF WORK :   40 hours per week and as needed, including late evenings and possible weekends.

    SALARY RANGE :   As specified in the Notice of Hire. Perform Other Duties as Assigned.

  • Days, accepts flexible schedule to meet department needs.
