2 recent Charge RN / Manager / Supervisor references matching specialty

Skills Checklist matching specialty

Drivers licenses with address that matches demographics

Current AHA BLS required

NURSYS Verification Report, pulled within the last 7 days, showing ACTIVE MA RN license; must show ALL licenses (past or present) on report

RTO must be included at time of submission; it will not be guaranteed after confirmation.

EMR Exp Required(or Preferred) : EPIC Flu vaccine required during season will not be guaranteed after confirmation.
