What You'll Do :

As an LPN, you'll play a vital role in supporting our patients and RN Case Managers by :

Following the individualized plan of care and physician's orders to deliver safe, high-quality nursing care.

Teaching patients and families simple self-care techniques to support independence.

Monitoring patient progress, reporting changes in condition, and suggesting updates to care plans.

Providing positive, compassionate communication to patients, families, and caregivers.

Helping prevent unnecessary ER visits or hospitalizations by delivering proactive care at home.

What You Need :

High School Diploma or GED

Graduate of an accredited LPN / LVN program

Current State License (LPN / LVN)

Minimum 1 year of clinical experience (home health or hospice a plus)

Reliable transportation, valid driver's license, and insurance

Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift to 50 - 100 lbs. as needed.

The base salary for this position is $25.00 to $37.00 per hour, based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.

This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and / or team performance.
