No APP coverage

2-3 providers at a time

6-8 patients per day

Provider Qualifications :

Board certified providers only (No BE)

Facility can privilege a clean provider in 10 days from application receipt

Submission Requirements :

Must be submitted at the time of name clear (Required, no exceptions)

Board Certified (No BE)

Active MI license (Not accepting IMLC, Required with no exceptions)

Clean background with no actions (Minimum malpractice is OK, Required)

Prior experience (Completed Fellowship with a minimum of 2-3 years, Required with no exceptions)
