About the Role

Are you a Certified or Registered Medical Assistant (CMA / RMA) with a passion for patient care and chronic disease management? We're seeking a Remote CCM Enrollment Coordinator to join our growing team.

In this role, you’ll be responsible for connecting patients to our Chronic Care Management (CCM) Program . You’ll identify eligible patients, obtain and document verbal consent, explain the program’s benefits, and develop initial care plans for provider review. You’ll also assess patient health, educate patients about their chronic conditions, and serve as a vital link between patients and providers—ensuring patients get the support they need.

What You’ll Do

• Conduct CCM verbal enrollments and determine patient eligibility.
• Obtain and document verbal consent for CCM services.
• Educate patients and families on chronic conditions, medications, and care expectations.
• Communicate effectively with providers, staff, and healthcare professionals.
• Promote adherence to individualized care plans.
• Increase patient self-management, assist with shared decision-making, and support patients in reaching established goals.
• Connect patients to relevant community resources to improve outcomes and satisfaction.
• Ensure compliance with CMS guidelines and healthcare regulations.
