SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

• Provide assessment services as needed; determine proper level of care.
• Provide aftercare planning services
• Maintain an active caseload of patients.
• Deliver therapeutic services including : individual, group, and family therapy sessions.
• Maintain required patient records including treatment plans, group and individual progress notes, and case management notes.
