SUMMARY : This position is responsible for providing substance use treatment services to individuals and groups at A Safe Haven's main campus.
DUTIES AND RESPONSIBILITIES :
- Completes intakes and assessments utilizing the ASAM Criteria to determine the appropriate level of care.
- Develops and reviews treatment plans, goals, and objectives with clients.
- Enters all required client data into the agency electronic health record system (AWARDS).
- Facilitates substance abuse education and treatment groups.
- Provides individual substance abuse counseling.
- Documents all provided services and maintain thorough and accurate records in accordance with DAD Rule 2060 and agency policies.
- Completes discharge summaries for clients.
- Conducts comprehensive chart reviews in preparation for annual audits.
- Participates in staff meetings and trainings.
- Performs other related duties as assigned by management.
QUALIFICATIONS :
Bachelor’s degree (B.A.) from four-year college or university in social work, psychology, or criminal justice related field preferred;
OR two years of related experience and / or training; or equivalent combination of education and experience
- Certificates, licenses and registrations required :
- CADC or MISA preferred OR obtained within two years of employment.

Monday thru Friday
8 am - 4 : 30 pm
