A well-established skilled nursing and rehabilitation center in the Wakefield MA area is seeking an experienced and detail-oriented MDS Coordinator to join our team full-time. The ideal candidate will be a licensed nurse with strong clinical assessment and documentation skills who can ensure accurate and timely completion of MDS assessments and compliance with all state and federal regulations.
Responsibilities :
Complete and coordinate MDS assessments in accordance with state and federal guidelines.
Ensure accurate coding and documentation to support reimbursement and quality measures.
Collaborate with interdisciplinary team members to develop and update care plans.
Monitor resident progress and maintain regulatory compliance.
Participate in care plan meetings and quality assurance programs.
Qualifications :
Current RN or LPN license in Massachusetts (required).
Minimum 1 year of MDS experience in a skilled nursing facility preferred.
Strong knowledge of RAI process, PDPM, and Medicare / Medicaid guidelines.
Excellent attention to detail, communication, and organizational skills.
Ability to work independently and as part of an interdisciplinary team.
