The Jail Care Coordinator embedded within the Medina County Jail is a dedicated individual in the Mental Health Grant Program for Justice-Involved Individuals (MHGJII). In this role, you will provide crisis intervention services as needed, along with providing planning and case management for inmates with mental health and/or substance use conditions who are eligible for pre- and post-release from the jail. This position would also act as a liaison between the County Jail, HCMHDD Centers, and local community resources to ensure continuity of care for individuals with the intention of reduction in recidivism rates related to mental health and substance use conditions. Duties include coordinating behavioral and physical health needs while in jail and upon release from jail; providing behavioral health consultation and follow up with primary care and behavioral health care patients. The delivery of services will be based on the needs of the community and individuals experiencing a mental health need. The Jail Care Coordinator will assess specific needs to be able to develop and implement Person Centered Recovery plans that support individuals with mental illness/substance use in learning skills to reach their specified service outcomes and to attain/maintain individual recovery. The Jail Care Coordinator will complete pre-intake needs and post-release assessments. The Jail Care Coordinator will follow cases for up to 90 days post-release to ensure connection to the appropriate level of care in the community. The Jail Care Coordinator must provide timely and accurate documentation that supports diagnosis and outcome-based services. This position collaborates with intra- and inter-agency staff to improve services for the community. The Jail Care Coordinator exercises extensive independent judgment and will help to maintain data, which will be used to plan and make projections for current and future services to meet outcomes. Duties of the position will require frequent interactions with jail staff, community members, Primary Care Physicians, hospital, and Hill Country MHDD program staff. Requires flexible hours, involves extensive paperwork, and will require self-directed performance. Must have a valid Texas driver’s license and a driving record that is acceptable for insurability through the Center’s insurance carrier, and pass a criminal background check. Will participate in community coalition meetings, trainings, and Criminal Justice training seminars as designated. Supervised by the MHDD Adult Mental Health Director.

Essential Duties and Responsibilities:

  • Assesses all individuals with positive jail screening and/or referred consumers from jail staff to determine appropriate referrals needed or if a doctor visit is recommended. Completes all needs assessments for incarcerated individuals within 3 business days. Ensures that all individuals have a current Uniform Assessment prior to release. Must provide collaborative documentation at least 95% of the time or no later than 24 hours after delivery of service. Will deliver services using the recommended Evidence-Based Practice recommended by HHSC. Person Centered Recovery Planning is developed in collaboration with the individual/family served and any other person or provider requested by the individual/family. Maintain grant data collection. Respond to individuals in mental health crisis by providing immediate emotional support and active listening. Establish a supportive and non-judgmental relationship with individuals in crisis, demonstrating empathy and understanding. Collaborate with mental health professionals, along with jail staff, to determine appropriate interventions and make referrals to appropriate services and resources within the community. Assist individuals in developing personalized crisis safety plans, fostering empowerment and self-management strategies to promote long-term mental health stability. Educate individuals on available community resources for ongoing support once released from jail. Maintain accurate and confidential records of interactions with individuals, adhering to organizational policies and legal requirements. Collect, analyze, and report on data necessary to contribute to the overall understanding and improvement of crisis response services. Stay updated on current mental health practices, crisis intervention techniques, and case management best practices. Participate in ongoing training and professional development opportunities to enhance your skills and knowledge. Work collaboratively with a multidisciplinary team, including mental health professionals, crisis counselors, law enforcement, probation, and parole officers. Engage in regular team meetings and case consultations to ensure coordinated care for individuals in crisis. Assess individual needs and collaborate with people to develop and execute effective recovery and life management plans. Provide relevant skills training to improve daily life. Collaborate with service providers and agencies to be a liaison for effective care and positive outcomes. Collaborate with and provide outreach to community partners, organizations, and stakeholders. Consistently meet with individuals to maintain progress as well as compliance. Work alongside law enforcement and emergency medical personnel. Assist in grant data tracking. Model effective coping techniques and self-help strategies. Maintains effective communication with all treatment staff, community stakeholders, and consumers.
