Nurse Practitioner Hospice (Providence Hospice Sonoma County)

Join Providence Hospice Sonoma County as a full-time nurse practitioner and help serve a patient population in need throughout Sonoma County in northern California.

This is an outpatient and in-home care role where you will have a large amount of autonomy while still being part of part of a supportive multidisciplinary team.

Monday-Friday 9 : 00 a.m. to 5 p.m., with weekend and evening call* Outpatient care, in-home visits* Compensation is between $162,757 and $210,093 per year* The posted salary reflects the starting range of total compensation and may include productivity bonuses, incentives related to quality and performance, hospital / ED call stipends, extra shift incentives, and other forms of cash compensation as applicable to the position.

In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. This well-established group collaborates closely with Providence Palliative Care as well as partners with local hospitals, assisted living facilities, skilled nursing facilities, and board and care facilities in order to provide full-spectrum care services to some of the most medically complex and vulnerable patients in the county.
