Care Coordinator

A company is looking for a Care Coordinator.

Key Responsibilities

Support patients in scheduling provider visits

Ensure proper documentation of all patient contacts

Answer patient calls and transfer escalated patient issues appropriately

Required Qualifications

High School Diploma / GED (or higher)

Experience with escalated customer service issue resolution

Knowledge of Microsoft Office products, including Word, Outlook, PowerPoint, and Excel

Phone / Fax system experience

EMR experience, with Athena & Epic preferred
