Job Description

Inova Fairfax Sickle Cell Team is looking for a dedicated Experienced Social Worker Case Manager 2 to join the Case Management Team. This role will be Full-Time, Monday-Friday 8 : 00am - 4 : 30pm. No evenings & no weekends. This role would split time working between the Inova Fairfax Campus and Inova Loudoun Campus.

The Sickle Cell Social Worker Case Manager 2 provides psychosocial and logistical support as a member of the multidisciplinary sickle cell team. This role ensures timely patient management and progression of care while serving as a key liaison between patients, families, and providers. Responsibilities include building relationships with community organizations, facilitating the monthly support group, providing limited short-term counseling as needed and connecting patients with external mental health resources, linking patients to resources addressing food, transportation, financial, and other social determinants of health, and assisting with blood drives and the annual town hall.
