As a Certified Community Behavioral Health Clinic, CNS Healthcare's mission is to serve the people of our communities. The Assertive Community Treatment (ACT) Case Manager serves as a member of a community-based mental health team that provides services for individuals experiencing serious mental illness. The mission of ACT is to help people become independent and integrate into the community as they experience recovery through a team-based service model. The ACT Case Manager will work with individuals and groups to promote optimum mental health. In addition, helps individuals deal with addictions and substance abuse, self-esteem, and issues associated with mental and behavioral health.
Please note : As a community-based role, this position will render at least 80% of services in the beneficiary's home or other agreed-upon community-based position.
Compensation Range :
The starting pay for this position is $44,000 for unlicensed candidates, $44,000 - $65,770 for limited licensed candidates, and $52,500- $75,300 annual salary for fully licensed candidates, and is based on non-discriminatory factors such as skills and experience.

Qualified Mental Health Professional (QMHP)Bachelor's Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or other Human Services field
