• Adheres to the standards of client care and implements the policy and procedures of the Agency.
• Performs client personal care, household services essential to client care at home.
• Reporting changes in client conditions
• Establishes a relationship with the client and family which transmits trust and confidentiality.
• Prepares report of visits daily and incorporates the same into the clinical record on a weekly basis.
• Reports any change in the client's mental or physical condition or in the home situation to the supervisor
• Performs other duties as necessary and requested
