Speech Therapist, is responsible for providing rehabilitation services for home based clients in need of speech therapy to assist patients in communicating and swallowing appropriately, including assessing patients, diagnosing disorders that can impact swallowing and speaking and providing treatment for their patients
Accepts responsibilities for all identified duties.
Visits patients in home / hospital. Performs initial and ongoing assessments, including Oasis assessments at appropriate time points and evaluates the level of function and identifies problems.
Develops initial and ongoing treatment plans, including goal setting
Provides treatment in accordance with physician orders.
Utilizes equipment required to increase the client's function and independence.
Re-evaluates patient as appropriate.
Revises plan as necessary.
Prepares clinical and progress notes in a timely manner.
Provides for discharge planning and communicate plans to case manager.
Consults with clients and families and VNA staff as appropriate.
Participates in bi-weekly IDT meetings as approiate.
Participates in Quality Improvement activities.
Makes appropriate interorganization and intraorganization referrals.
Participates in inservice programs as required.
Familiarizes and complies with all VNA policies and procedures.
Meets standards of behavior expectations.
Follows established guidelines for use and / or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy immediately
Other duties as assigned.
Compensation : $80.00 per visit, depending on experience.
Available Employee Benefits : 403b (Only eligible if 1,000 or more hours worked per year)
Graduate from accredited school with a master's or doctoral degree in speech-language pathology; Meets the education and experience requirements for a Certificate of Clinical Competence (in speech pathology or audiology) granted by the American Speech-Language-Hearing Association.
Licensed in the State of Illinois as a SLP
Maintains current CPR certification.
Health policies and requirements met (see personnel policies).
Three years experience as SPL preferred.
Computer literacy.
