- Must Currently be a hospice marketing rep in San Antonio to qualify for interview
We are a Hospice and Primary group in San Antonio and have locations in Nevada and Utah. As a marketing liaison you will be responsible for facilitating relationships with prospective patients, physicians, and other referral sources interested in hospice care. You are responsible for upholding the organization’s mission to deliver exceptional care by driving referral growth and maintaining strong relationships with referral partners and the community. This role requires a commitment to representing our organization professionally, educating the community, and ensuring our organization remains a trusted resource for patients, their families, and our community partners.
Duties include, but not limited to
Referral relationship managementCompliance and professional standardsGenerate and manage appropriate Hospice and Home Health referrals / admissions from accounts.Processes new referrals and ensures all necessary information, i.e., H&P, Orders, Demographics, Rx is available.Coordinates referrals from the doctor’s office, hospitals, skilled nursing facilities, rehabilitation centers, assisted living facilities, etc. to the office.Establish and maintain market awareness, educate on HospiceDelivers orders to physicians for signatures.Liaison between external referral sources and internal team.Observes confidentiality and safeguards patient-related information.Provide effective solutions to develop new businessResponsible for maintaining momentum of geographical areaAchieve referral goals set forth by the company and self per month.Meet or exceed established referral and admission targets.Ensure patient satisfaction while developing business growthDemonstrate effective time management and maintain a consistent schedule of outreach and follow ups.Provide accurate and timely reporting on referral activities and market share feedback.Maintain professionalism, compassion, and a patient centered approach in all interactionsOther duties as assigned.Benefits :
