Overview
Assist with adults with I / DD disability in a day program setting. Implement individual Person Centered Plan involving skills that promote independence while instructing, modeling, assisting the consumer in daily living skills, socialization, leisure and / or recreational activities, educational and vocational skills.
Responsibilities
- Receive, read, and be familiar with the Employee Handbook regarding Personnel Policies and Procedures for the agency.
- Demonstrate knowledge, skills, abilities, and consumer competencies, including knowledge of basic principles of I / DD, and ability to work with persons with varying abilities, ages, economic status, and backgrounds.
- Become familiar with and implement the individual's habilitation plan, such as the Individual Support Plan, or address identified goals according to the service definition.
- Document the service provided in progress notes, including observations, consumer participation, progress, behavior changes, and any pertinent information. All documentation must be completed on the day the service is provided and in BLACK INK only.
- Adhere to policies that prohibit taking a person supported to a staff's private residence or leaving a consumer unattended in a vehicle or in any other public place.
- Submit all required documentation by deadlines specified by the Supervisor.
- Maintain current all training required by the agency, according to the service provided and the individual's needs; failure to renew training by due date may result in removal from schedule until training is up-to-date.
- Attend all mandatory staff meetings and consumer conferences or notify the Supervisor in advance of inability to attend.
- Receive supervision from the Supervisor / Qualified Professional (QP) or Associate Professionals (AP) as of waiver implementation, at least quarterly and as needed via face-to-face, phone, or group staff meetings.
- Be supervised by a Qualified Professional in accordance with applicable licensure or certification requirements.
- Work only during authorized hours unless specifically approved in advance by the Supervisor / Administrator.
- Report changes in a person's condition, family situation, or needs to the Supervisor promptly.
- Report any allegation of consumer abuse, neglect, or exploitation to the Supervisor to be investigated and an incident report completed, along with a report to the local DSS.
- Report any destruction or theft of agency property to the Supervisor.
- Report any possible fraud against the consumer or agency to the Supervisor to be investigated.
- Perform other duties as required by circumstances.
